Job description

- Bread and butter primary care
- Clinic is open M-F 8-5
- No call or weekends
- Mostly sick and well visits with some chronic disease management
- Must be able to see all ages
- Can license if needed for the opportunity but would prefer providers that are already licensed in IN, IL, or WI
