Coverage Dates: ASAP start – 3 month contract w/ possible extension options
Credentialing Timeframe: 2-3 weeks+

Schedule: Monday-Friday, 8a-5p
Bread and Butter Community Health – Outpatient Primary Care & Urgent Care
Treating ALL AGES
18-22 patients per day (with support staff)
