Details:
• Board Certified - REQUIRED, will possibly consider truly BE (0-5 yrs of completion of residency)
• Active CA License - REQUIRED
• Clean malpractice/background - HIGHLY PREFERRED
• In additional submission details: Disclose if the provider has worked at any other CHI/Dignity Facility - REQUIRED, Note at name clear
• COVID VACCINATED - REQUIRED, note at name clear
• Long term coverage for 6 months with option to extend!
• Schedule: 40 hours per week / Preferably M-F 8 hr days / will also accept 4 days per week at 10 hours / No on-call
• Family Practice or Internal Medicine Physician
• Shift type: Hourly
• Setting: Outpatient family practice
• Family practice patients, some acute care walk in
• Required Procedures: Joint injection, trigger point, preferred but not required
• Types of cases: Chronic care, multiple complexity, high volume of diabetic, HTN, COPD, CHF
• Reason for coverage: Loss of employed FNP, looking for physician to cover due to the complexity of our patients.
• Rural Health Clinic, open 5 years offering: Primary Care, Acute Care, Pediatrics, Podiatry
• Medical Director will be on-site 6 months out of the year.
• Patient volume expected 18 minimum, higher if acute care is scheduled.
• EMR: Cerner Ambulatory
• Support Staff: Medical Assistant
• Credentialing timeframe: 30 days minimum
Shift Types:
• 5 days per week, 8 hour days would be: 8a-5p
• 8am arrival, patient starts at 8:30am
• Last patient at 4pm.
• 4-10s schedule is 7:30a-5:30p
• First patient at 8a
• Last patient at 4:40p
