Family Medicine Physician needed starting as soon as credentialed near Collinsville, CT

Job description

- Work Dates Needed: ASAP-Ongoing
- Preferred Schedule: Monday – Friday, 8a-5p
- Worksite Setting: Outpatient
- Scope of Work: Provide comprehensive medical services for adult members of family; no peds.
- Services include primary care and treatment, Preventative care, Immunizations, health education, prescribing of medications, ordering, or interpreting lab testing, physical examinations, STD screenings, Vision/hearing, family planning, and nutrition.
- 16-24 patients per day.
- Licenses, Certifications, Requirements: CT License, DEA, BLS; BC/BE, EPIC Experience Required
- EMR: Epic
- Credentialing: 60-90 days; temps avail w clean file
- $500 referral bonus received for any referral who works 30 days.
