• Setting: Primary Care
• FTE: Full Time
• # Providers Needed: 1
• Duties: Primary Care - General
• Required Procedures: Not required, whatever the provider feels comfortable with
• EMR: Cerner
• Patients per day: varied, about 18. 15 minutes with follow up patients, 30 minutes with new patients
• Support Staff: RN, MA
• Worksite Resources: Labs/X rays off site
• Reason for Coverage: busy clinic, coverage for physician who left

Credentialing Information:
• Timeframe: 2-3 months
• Requirements: BE/BC in FM, active CA license
