Job description

I'm working on a full-time permanent geriatric primary care physician position in Kentucky. It's a facility that supports those that may require nursing level care remain in their home. Part of a larger organization with 10+ locations nationwide.


Key Responsibilities:

-Perform Comprehensive Assessments: Conduct thorough history and physical exams for new referrals.
-Interval Assessments: Perform periodic history and physical exams for established participants, completed quarterly or as needed.
-Episodic Care: Evaluate and treat participants with acute illnesses.
-Specialist Referrals: Refer participants to medical specialists as necessary.
-Hospital Admissions: Admit participants to the hospital when required and provide primary care management, update the IDT regularly, utilize specialty consultants judiciously, and coordinate discharge planning with the IDT.
-Nursing Home Care: Manage care for participants in nursing homes by providing regular visits, communicating with nursing home staff, and admitting participants to the hospital when necessary.
Family Conferences: Actively participate in family conferences regarding care planning and implementation.
-24-Hour Care Coordination: Oversee the coordination of 24-hour care delivery.
-Documentation: Accurately document participant changes in medical records and communicate updates to the IDT promptly.
-Interdisciplinary Team Participation: Participate as an IDT member, attending staff meetings and contributing to participant care planning.
-Continuous Education: Maintain and enhance education regarding quality clinical standards and best practices for geriatric care.
-Committee Involvement: Participate in Ethics and Quality Improvement -Committees and/or work groups as assigned.
