Provider Profession Needed: FM MD/DO - must see all ages including newborn

Start Date: ASAC - takes 2 weeks

End Date: 3 months

Length of Assignment: 3 months plus extension

Credentialing Timeframe: 2 weeks

Work Schedule: M-F 8-5p

Patient Volume: 26 patients scheduled, 18-20 average ppd

Procedures :Family Medicine, Primary Care, Preventable Health, Immunizations, Wellness Checks, newborns

EMR: Nextgen

License Needed: CA

Certifications Needed: BC or BE

Relevant Experience/Skill Set Needed: FQHC experience and nextgen

Other additional pertinent information: No GP
