NP Needed Part-time for At-Home Visits

Job description

• In Home Primary Care Visits
• Diabetes, Weight Management, Hypertension
• Wound Care experience is a plus, not mandatory
• These patients are referrals from the Wound Care providers
• Provider will NOT go to zipcodes deemed unsafe or even unfavorable
• Typical day starts at 7:30 AM to 5:30 PM
• AdvancedMD is the EMR, provider will be given an iPad.
• Provider will be driving his/her own vehicle - CLIENT WILL REIMBURSE MILEAGE
• 2 Days Per Week, flexible on the days
• 15-30 Day Credential
