About the Job
The Nurse Practitioner (NP) delivers advanced wound care in a mobile, field-based environment, serving patients across skilled nursing facilities, assisted living communities, and home settings. Working autonomously while supported by UWI’s multi-disciplinary clinical leadership, the NP is the primary provider in the field, ensuring consistent, high-quality wound management and driving improved healing outcomes in alignment with UWI’s national standards.
Key Responsibilities
- Conduct comprehensive patient and wound assessments, staging, and treatment planning.
- Perform wound-care procedures including sharp debridement, dressing changes, compression therapy, and application of advanced treatment modalities.
- Deliver home-based wound care services, providing treatment directly in the patient’s residence or designated care setting.
- Support and apply advanced wound modalities, including artificial, stem-cell, and growth factor–based skin substitutes, ensuring appropriate preparation and utilization.
- Provide education to patients, caregivers, and facility staff on wound management and prevention strategies.
- Collaborate with supervising or consulting physicians and engage with regional clinical and operations teams, leveraging a multi-disciplinary support model to ensure continuity of care and exceptional clinical outcomes.
- Maintain timely and accurate documentation within the EMR system.
- Travel to multiple patient locations as needed within the assigned territory.
- Ensure adherence to infection control protocols, safety requirements, and clinical quality standards.
Skills, Knowledge, and Expertise
- Active Nurse Practitioner (APRN/NP) license in the state of practice.
- Master’s Degree in Nursing from an accredited institution.
- Prior wound care and sharp debridement experience highly preferred.
- Current BLS certification required; ACLS a plus.
- Board Certification through ANCC or AANP preferred.
- Valid driver’s license and reliable transportation to travel between patient locations and provide care in home and post-acute settings.
- Proven ability to work independently in a mobile, field-based role while collaborating with a multi-disciplinary clinical team.
