Schedule: Full-time weekdays; choose between 5×8-hour shifts or 4×10-hour shifts, with somewhat flexible start/end times
Patient Volume: 5–8 patients per day
Work: Home visits focused on primary care, Transition Care Management, Chronic Care Management, wellness visits, and other primary care services
Coverage Area: Typically about a 10-mile radius from the clinic
