🚨 Home Health / Primary Care Locum – Annandale, VA

📍 Annandale, VA
📅 Start: Sept/Oct | Acute need
📋 3–6 month assignment
⏰ Full-Time Weekdays | 5x8s or 4x10s
⭐ Flexible start/end times
🏠 Home-Based Primary Care + Clinic
💻 eClinicalWorks (eCW)
🎓 New Grads Welcome & Preferred
📍 Local candidates preferred
💰 Paid onboarding/training

Clinical Details:

* Home visits: Approximately 5–8 patients/day
* Clinic: Approximately 3–4 patients/hour
* Home visits generally within a 10–20 mile radius
* Comprehensive primary care
* Transition Care Management (TCM)
* Chronic Care Management (CCM)
* Wellness visits
* Value-based care model
* Collaboration with Remote Patient Monitoring, mental health & endocrinology programs

Requirements:

* Comfortable providing comprehensive primary care
* Ability to efficiently navigate eClinicalWorks
* New graduates encouraged to apply
