• Is CRNA supervision required? Yes
• Clinic schedule (days and hours)? Each Shift is 24 hours 7a to 7a
o How many clinic patients/day? No Clinic—Strictly OR
• Will the physician be on call? Yes
o Please describe (week night, weekend, phone call only, ER back up, Hospital)? ER/OR/OB
o What is the Call Schedule? When they work they are first Call
