Social Worker – Dialysis

JOB DESCRIPTION

✓ Provide emotional, social, and practical support to patients and families managing chronic kidney disease and dialysis treatment
✓ Help patients and families navigate the challenges associated with chronic kidney disease and ongoing treatment
✓ Collaborate with the healthcare team to support patient well-being and continuity of care

SUBMISSION REQUIREMENTS

✓ 2 years of Social Worker experience – REQUIRED
✓ Dialysis experience – REQUIRED
✓ Active Michigan license at time of submission – REQUIRED

PROFILE REQUIREMENTS

✓ Current work history
✓ Skills Checklist completed within the last 1 year and specific to Social Worker– REQUIRED
✓ 2 Supervisory Reference Forms completed within the last 2 years – REQUIRED
✓ Active American Heart Association BLS – REQUIRED

STATE LICENSE REQUIREMENTS

✓ Michigan License Required
✓ License must be active at the time of submission
