Apply social work theory, knowledge and methods to improve the social condition of individuals, families through the provision of services other than counseling or Sign up or sign in for contact information. Obtain information from agency clients and/or applicants for services to determine their service needs and/or the availability of appropriate Sign up or sign in for contact information. Assist individuals, families, and or groups in identifying, locating, arranging, and making use of resources and services that are available to resolve day-to-day problems (e.g., employment, housing, health care, financial assistance problems, placement resources, etc.).4. Assist individuals, families in long term planning and assistance for decision Sign up or sign in for contact information. Provide information on Assisted Living/Nursing home care/placement Sign up or sign in for contact information. Evaluate patient progress towards goals and advise care plan accordingly in consultation with appropriate individuals, team members and/or social work Sign up or sign in for contact information. Work collaboratively in communication with multidisciplinary team to support care plan and team Sign up or sign in for contact information. All documentation meets the needs of Medicare, DPH and agency requirements and is completed and transmitted within 24 hours according to Sign up or sign in for contact information. Maintain current knowledge base on social work issues through conferences, educational requirements for licensure, and social work Sign up or sign in for contact information. Perform miscellaneous job-related duties as assigned.*Qualifications:** Minimum of one to two years as a social worker in acute medical or related health care setting.
