Locums – Home Health / Telehealth NP | Greensboro, NC

Assignment Details

Location: Greensboro, NC (travel to surrounding 5 counties)

Start Date: November 3, 2025 (ASAP)

Contract Length: 12 weeks, option to extend

Schedule: Monday–Friday, 8:00a–5:00p (40 hrs/week)

Coverage: Scheduled, no call

Setting: Home Health with Telemedicine

EMR: Athena (experience preferred or must complete training modules before start)

Work Details

Coverage needed: 1 FTE

Patient load:
• 6 patients/day for home visits (avg. 45 min appointments)
• 8 patients/day for telehealth visits

Population: Geriatric, mix of new and established patients (many with multiple comorbidities)

Responsibilities include:
• Physical exams, chronic disease management, acute illness care
• Advanced care planning & patient education
• Coordinating care with interdisciplinary team

Scheduler provides confirmed visits the day before

All telehealth equipment provided

Requirements

Board Certification: FNP or Adult NP

NC License & NC DEA required

BLS (AHA or Red Cross) required

Valid driver’s license, personal vehicle, and auto insurance required

2–3 years of experience in acute care, home health, long-term care, or outpatient setting required

Palliative care experience preferred

Additional Notes

Providers will be credentialed under the CMO’s license until payer credentialing is complete

Must be comfortable with complex patients (some with up to 10+ comorbidities)

Documentation must meet compliance standards and be thorough
