Hospice RN Case Manager needed in San Diego, CA

Job description

The Hospice RN Case Manager is responsible for planning, organizing, and directing hospice care for patients in their homes and communities. Key duties include conducting comprehensive patient and family assessments, developing individualized care plans in collaboration with the interdisciplinary team, administering medications and treatments as prescribed, educating patients and families about end-of-life options, and ensuring accurate documentation. The RN Case Manager coordinates care with physicians, social workers, aides, and other team members, supervises hospice aides, and participates in on-call rotations as needed. The position requires travel for patient visits within the San Diego area and a commitment to compassionate, patient-centered care
