Licensure: Active Arizona medical license or IMLC accepted
Requirements: Board certified or board eligible in Internal Medicine or Family Practice; minimum of 2 years of experience required
Certifications: BLS required
DEA: Required
CSR: Not required
Credentialing: Approximately 30 days
