No Call or weekends
$125k to start plus bonus potential

We are looking for a motivated and experienced Nurse Practitioner to join us full time in Florham Park, NJ. The Nurse Practitioner will be providing inpatient medical coverage at Skilled Nursing Facility and will be working alongside a medical director but will primarily be independent throughout the day so must be confident in ability to practice medicine. The incoming Nurse Practitioner must have inpatient medical experience, internal medicine and/or SNF experience is also a plus. We promote an excellent work life balance with no call or weekends!

Job Responsibilities:
• Perform appropriate follow-up medical care throughout the SNF/rehabilitation stay.
• Round on patients, medication management, track patient progress, remove obstacles to functional progress, and improve quality of life.
• Attend to caseload by having acute awareness of all referral sources and potential referral sources
• Maintain an active patient list and diligently capture all referrals.
• Engage with clinical staff on a regular basis and maintain a professional, collegial relationship with other facility physicians, caregivers, administrators, and directors.
• Engage with patients and families during care and communicate clearly to promote understanding of treatment options and procedures. Set expectations with facility, patients, and families.
