Practice Setting: Office-Clinic
Schedule (Shifts/Hours): 8:30 - 5:00 M-Th; 8:30 - 12:30 F
Average Number of Patients per Day (Private Practice): 15-20
Documentation System/EMR: Epic
Number of Exam Rooms: 6
Number of Rheumatologist in Practice: 1
Description of Support Staff: 1 NP, 2 CMAs, 1 front desk staff
Patient Age Demographic: Adult
0 % Internal Medicine, 100% Arthritis, Autoimmune

Required Procedures:
Joint Injections - Hyalgan
Joint Injections - Supartz
Joint Injections - Corticosteroids

Submission Requirements
Board Certified - REQUIRED
Active SC license - REQUIRED, no exceptions
BLS - REQUIRED
depending on situation
No new grads, must have great experience, ratings, and training
Provider must be local - REQUIRED
